At the end turn right (THIS IS SECTION 4A of the HOSPITAL BUILDING) 6. upon entering 4A which is a ward the location of RENAP OFFICE is behind the nurses' station 7. head back the same way you came from 8. get back the ID you left at the EXIT guard HIP REQUIREMENTS: 1. create a list of references - personal or work related 2. Provide your PRC number 3. PAY 300 pesos 4. Validity of the ID is 1 year 5. You then will receive a hard paper card like ID wherein you will fill-in your name & signature
