PURPOSE AND SCOPE
The purpose of this document is to spell out the action to be taken by Premala Construction Sdn Bhd (herein after will be referred as PCSB) to ensure that the scaffolding activities are carried out systematically in a safe and healthy manner. outline the health, safety and environment (HSE) that shall be implemented by Premala Construction Sdn in Sabah Ammonia Urea Project (SAMUR) as per requested by the client and MHI/APEX/REKIND (Hereinafter referred as CONSORTIUM). It also served as a conceptual framework of its overall HSE activities in this site. 2.0
DEFINITION
HSE – Health, Safety and Environment PCSB – Premala Construction Sdn Bhd CONSORTIUM – MHI/APEX/REKIND SAMUR – Sabah Ammonia Urea Project Subcontractor – a person, party, company or partnership who has entered into a contract for the purpose of erection, modification and dismantle of scaffold on a sub-contract basis under Premala OSHA – Occupational Safety and Health Act 1994 FMA – Factory and Machinery act 1967 3.0

PCSB policy on occupational safety and health provides an overall sense of direction and sets the principle of action related to HSE which are to be integrated into the overall of business process. Our HSE policy statement can be referred in appendix 1 5.0
HSE ORGANIZATION
PCSB management team has established HSE committee in order to comply with the respective local legislation, OSHA 1994 and the hierarchy are as follows : CHAIRMAN Project Manager

The function of the HSE committee are as per regulation 11, occupational Safety and Health (Safety and Health Committee) 1996 as below : a) Assist in development of safety and health rules and safe system of work b) Review the effectiveness of safety and health programs c) Carry out studies on the trends of accident, near miss accident, dangerous occurrence, occupational poisoning or occupational disease which occurs at the place of work, and shall report to the employer of any unsafe and unhealthy condition or practices at the place of work together with recommendation for corrective actions. d) Review the safety and health policy at the place of work and make recommendation for the employer for any revision of such policies 6.0
RESPONSIBILITY AND ABILITY
The corporate structure of the company used in this project are as attached in appendix 2. The job description of the company’s personel in order to achieve the HSE target and objective are as below : NO 1

a) To aid the mobilization of new manpower in term of HSE induction and registration of workers with the consortium. b) To do daily HSE inspection c) To do hazard identification, risk assessment and suggestion of risk control d) To be the secretary of HSE committee e) To advise subcontractor under PCSB pertaining HSE f) To aid PCSB in complying with the project HSE requirement and the local law and legislation a) To take the main role in disseminating HSE awareness and any communication pertaining HSE b) To ensure all workers under his/her supervision follow the correct safe procedure for scaffolding operation c) To assist PCSB in achieving its HSE objective and target a) To follow all rules regarding HSE that have been made by PCSB b) To wear PPE at all time during operation commence c) To follow all the safe working operation of scaffolding d) To maintain good material arrangement e) To participate in any PCSB HSE program
COMPLIANCE TO RELEVANT LAW AND LEGISLATION
PCSB are committed in fulfilling the requirement / comply with the relevant local laws and legislation on occupational safety and health but not limited to the following : a) Occupational safety and health act, 1994 and its regulation b) Factories and machineries act 1967 and its regulation, especially Factories and machinery (Building Operations and Works of Engineering control) Regulation, 1986 PCSB has made their legal and commit to comply and maintain following the requirement of applicable law and legislation. 8.0
HSE TRAINING
All employee shall be provided with the relevant information on the occupational safety and health law and operation training upon commencing the job. The area of concern shall be of the following but will not limited to its : a) b) c) d) e) f) g)

All training shall be done in-house and other training required by the consortium shall be under their responsibilities. A training matrix shall be made available by HSE representative 8.1
HSE Induction
All employee shall undergo HSE induction done by consortium before or upon entering the site to work. 9.0
HAZARD IDENTIFICATION, RISK ASSESSMENT AND RISK CONTROL
At the outset of scaffolding works, PCSB have identify potential hazards and risk and accordingly develop a comprehensive plan to prevent, eliminate these risks and hazard associated with its operation. The hazard shall be categorized into specific category either quantitatively or qualitatively. A sample of its risk matriz is shown as below

After risk assessment has been done, risk control shall be implemented according to the below risk control measure hierarchy : a) b) c) d) e) f) 9.0

To achieve the specific HSE objective and target, PCSB has outline its own HSE program and can be referred to the Project HSE program (document no. HSE/SAMUR/PROG) 10.
INCIDENT MANAGEMENT
Emergency situation such as fire, explosion, accident may occur anytime throughout our operation period. A general method of statement for emergency has been made and shall be made available for current and new employee. 10.1
Incident notification
PCSB shall notify the consortium as soon as possible with any mean of communication if theres any incident occurrence including near misses. All PCSB personnel shall be trained on how to notify any incident.

Incident investigation
Investigation shall be carried out by PCSB HSE committee and any result from the investigation shall be submitted to consortium and if needed a statutory reporting shall be made to Malaysia Department of Occupational Safety and Health. 11.
EMERGENCY RESPONSE AND PREPAREDNESS
A suitable emergency response plan to handle any emergencies that may arise shall be made available. Basic emergency training related to PCSB scope of work shall be done such as first aid training while fire evacuation drill shall be done following the consortium. A fire fighting equipment shall be made available to fire prone area such as PCSB site office. 12.
SUBCONTRACTOR MANAGEMENT
All subcontractor under PCSB shall be made aware of PCSB commitment achieving its HSE objectives and target. Penalty shall be made for any law and legislation violation. The work leader shall be the HSE representative for each subcontractor working for PCSB. 13.
PERFORMANCE MEASUREMENT AND MONITORING
A mean of performance measurement shall be available in its organization. The measurements are comprise of proactive measurement and reactive measurement. The proactive measurement shall include : a) b) c) d) e)

CONTROL OF RECORDS
Any HSE record shall be made legible, identifiable, and traceable. The records shall be stored, maintained and protected against damage, deterioration and loss. Where there are legislative requirement and regulatory requirement for recordkeeping, such requirement shall be complied with. 15.
MANAGEMENT REVIEW
The management shall review the policy, objective, target and any other HSE related matter for PCSB’ commitment for continual improvement.
