Petitioner, due to an honest mistake on her part about the settings of the hearings in this case per the Order of 14 March 2016 of this Honorable Court, was unable to attend the hearing set on 19 April 2016 since she reported to work on that day. 2. The Honorable Presiding Judge Elisa R. Sarmiento-Flores then required the submission of petitioner’s written explanation about her absence within five (5) days reckoned from 19 April 2016, or until 24 April 2016. In respectful compliance, attached herewith as Annex “A” is petitioner’s written explanation. 1

In addition, petitioner wishes to invite the attention of this Honorable Court to the typographical errors in her Judicial Affidavit, specifically referring to the statements found in her answers to Questions Nos. 9 and 38 wherein it was stated that the Certificate of Live Birth with Registry Number 8962240 was issued by the “Civil Registry of Pasig City” when it should have stated instead that it was issued by the “Civil Registry of Manila City.” On this matter, petitioner once more begs the kind understanding of this Honorable Court for such inadvertences and prays that such erroneous statements in petitioner’s Judicial Affidavit be corrected to reflect thereon the true and actual Civil Registry that issued said Certificate of Live Birth. PRAYER WHEREFORE, premises considered, it is respectfully prayed of this Honorable Court that: 1. The instant Manifestation/Compliance be NOTED; 2. Petitioner’s written explanation be deemed to have sufficiently addressed her absence during the 19 April 2016 hearing; and 3. The erroneous entries in petitioner’s answers to Questions Nos. 9 and 38 pertaining to the issuing authority of Certificate of Live Birth with Registry Number 8962240 be corrected from “Civil Registry of Pasig City” to “Civil Registry of Manila City.” Other reliefs, just and equitable under the premises are likewise prayed for.
