1.0
Purpose. The purpose of this procedure is to provide an interface methodology between the HAZCON Base in Calabar & client for dealing with emergencies, accidents and incidents during the execution of Project in the workshop.
2.0
Scope. This procedure applies to all emergency and incident response interfaces in of all company activities within Calabar fabrication yard. In the event of a major incident or emergency during the implementation of project activities, HAZCON NIGERIA LTD shall take primacy in the coordination and application of activities. At all times, HAZCON NIGERIA LTD shall maintain command and control, of all incidents or emergencies.

Any undesired circumstances that give rise to ill health or injury, damage to property, plant or the environment. Incident includes all undesired circumstances and near misses that could cause accidents.
MHSW Management of Health & Safety at Work Regulation setting out the legal requirements that an employer must implement with respect to the management of & Safety for their employees. HSW Health & Safety at Work Act setting out the duties of employers and employees with a view to securing the health, safety & welfare of persons at work. 4.1
Emergency Classification. Class A. An incident, which can be dealt with by on-site location personnel and resources. • The incident has no effect outside the site. • No serious danger to life, the environment or to ZIENGOF or CLIENT assets or reputation. • The incident will be managed by implementation of this Emergency Response Procedure. Class B. An incident which may be dealt with locally but requires involvement of external agencies.

The incident may be “on site”, have some effect outside the site or be “offsite” and external emergency services will be involved. There is likely to be danger to life, to the environment or to ZIENGOF or CLIENT assets or reputation. The incident will be managed by the emergency response team using this Emergency Response Procedure.
Class C. A serious incident on site which requires the involvement of wide range of HAZCON or CLIENT resources and government and external agencies. The incident will have technical, press, public affairs and personnel implications. These will be one or combination of the following: • Death and/or serious injury. • Potential for significant pollution or environment damage. • Substantial damage to property. A Crisis Management Plan may be activated at corporate level where it is deemed necessary. 5.0
Responsibilities. The response roles detailed below are defined to deal with all forms of potential incident and allow for the most extreme outcomes with HAZCON staff available at the Calabar fabrication yard. They are intended to be flexible in use to accommodate the level of variance in the event of incidents or emergencies occurring, and particularly offer those responsible for managing such events the ability to combine roles to suit the number and competence of staff immediately available.

Safety Officer. In the event of an incident or emergency, this role will keep the Project Manager informed with regard to the condition of injured personnel or the status of any incident or emergency. Responsibilities are: • As soon as an incident or emergency situation is reported, goes to the scene to evaluate and confirm that an incident or emergency situation has occurred or has the potential to occur. • Informs the Project Manager of incident or emergency status. • Secures and makes safe the incident or accident area. • Coordinates medical attention and assessment for any injured personnel. • Initiates a chronological log of events to provide a record of the details of any incident/emergency that may arise or has the potential to arise. • Take photographs (if possible) of the incident area.

Project Manager Responsibilities are: • Immediately assume the role of On Scene Commander and initiate the communication link with the HSE team Retainership clinic. • Release required resources • Request all necessary assistance and to preserve life and safeguard personnel. Medic. This role provides front end medical assistance to casualties in the event of an incident or emergency Responsibilities are: • Report to the On Scene Commander for further instructions. • Liaise with onshore medical team when required. • Provide initial first aid where required. • Initiate medevac where the need arises in consultation with On Scene Commander.

Actions for Persons Encountering an Incident or Emergency. • Raise alarm by shouting. • Attempt to extinguish fire only if safe to do so. • Care for the injured. • Proceed to muster point when instructed.
6.3
Action for All on Hearing the Alarms. • Stop all operations. • Make all work equipment safe by isolating from power sources. • Make the work area safe. • Proceed immediately to the allocated muster point using the nearest safe exit. • Await further instructions.

Action on Discovering or Suspecting Community Disturbance. Community disturbance is an act of attack on project personnel or assets by local community people with the view to disrupt project activities. Where community disturbances arise all attempts shall be made to resolve the issues through dialogue with community leaders. In the event of community disturbance personnel should: • Inform the project manager immediately • Alert other personnel in the vicinity. • Not offer resistance to demands. • Not get emotional. • Not dispute any issues with those involved in any disturbance. • Obey the Project Managers, instructions.

In the event that a demonstration, riot, gathering of people, happens at fabrication yard in Calabar every effort must be made to prevent access or entry of unauthorized persons into the work areas. 6.5
Action on Discovering Electrocution. Anyone discovering someone suspected or confirmed as having been electrocuted should take the following action. • Immediately raise alarm. • Seek assistance from other team to prevent others being affected. • Do not physically touch the casualty. • If possible identify & isolate the electricity supply by switching off at source. • Attempt to remove the casualty from the source of electricity using wood or plastic implements. • If safe to do so, establish if the casualty is breathing. • If possible, ister cardio pulmonary resuscitation (R mouth to mouth breathing and chest pumping) until medical assistance arrives.
